Our Policies

Our phone line is available Monday through Friday, 9:00 AM through 4:30 PM. We are available in the office by appointment only.  Should a question arise about treatment, payments, or insurance, please feel free to call.

Cancellations

We request at least one day's notice of any cancellation or change of an office appointment. Surgical cancellations should be limited to an emergency only and we request at least two days notice due to the amount of time required to coordinate scheduling.
 

Pre-Certification/Referral/Authorizations


Most insurance companies require notification (pre-certification) for surgery center or hospital outpatient procedures. If you are unsure about your policy or referral requirements, please contact your insurance company or benefits manager. We routinely reach out to your private payor to attain pre-certification as well.

Patient Privacy


We make every effort to ensure that all patient records are kept confidential. Our employees are prohibited from looking at patient records unless they are to provide services to our patients. All electronic billing complies with privacy provisions of federal law (HIPPA).
